Welcome to the 2008 MAPS Futsal Leagues and Youth Tournaments. We are looking forward to a great winter of competition.

Registration will be prior to your first game. Please arrive at least 45 minutes before your first game to provide adequate time to process these matters.

Each team will need to present a Player Registration & Release form for each player and a Team Roster. You can print the forms from the web.

The Roster can be accessed from the website, or directly at http://www.socceragency.net/njfutsal/2008/08_NJ_Roster.pdf.

Adult players must fill out and sign the USFF Adult Player Registration form. Download and print it from: http://www.futsal.org/publications/amateur.pdf.

Players who have not reached their 18^th Birthday must fill out the USFF Youth Player Registration form found at http://www.futsal.org/publications/youth.pdf, sign it, and have their parent or guardian sign it.

Teams that already have rosters and individual USFF player registrations filled out for participation in other recognized NJ Futsal Leagues may use those forms.

You can add players to the Roster during the course of the season, but rosters will be frozen ten days before the start of the League Playoff weekend. Playoff participants must be listed on their team’s Roster, and have participated in at least one weekend’s regular season games, in order to be eligible to participate in Playoff games.

For players not currently registered with US Futsal, the player registration fee is $4 per player to participate in the MAPS League; make your check payable to “NJ Futsal”. We are using a streamlined registration process at a reduced registration cost for those initially participating solely in the League, but teams that enter the State or Regional championships or who will be playing in other full-time leagues will need to undertake the full player carding process and pay an additional $3/player. (A permanent player pass for $7.00 covers the player for all Futsal activities for the entire seasonal year.) You will need to bring a facial photograph (face must fill a square inch) when you apply for the permanent pass.
